Starting September 28, 2011, all interested PhilHealth members and non-members can avail of themselves the New PhilHealth Card at all SM Branches nation-wide. With this new card, members do not need to be hospitalized in order to avail of incentives and benefits because this card is jam-packed with incentives like discounts from selected commercial establishments.
Availing the card is so easy. For new applicant, he is required only to fill-out the PMRF (PhilHealth Member Registration Form), attach supporting document like birth certificate or any valid ID and pay a minimal fee. For declaring qualified dependents, the documentary requirements should also be submitted.
The members will be notified within a month if the card is already available for pick-up.
Always remember, “Basta Kalusugan ng Pamilya, i-PhilHealth mo na!”
